Overview

While every lawsuit is unique, the documentation errors that commonly contribute to malpractice claims are both foreseeable and preventable.  By learning to recognize the mistakes that commonly contribute to malpractice suits, physicians should learn to document in ways that both improve patient care and reduce their own potential liability. 

Objectives

Upon completion of this course, a physician should be able to:

1. Explain why poor documentation is a major factor in many “failure to diagnose” and other malpractice claims.
2. List the elements of the LAWSUIT documentation model, and explain why each is important.
3. List billing practices that will likely lead to problems with Medicare/Medicaid as well as private insurers.
4. Explain the do’s and don’ts of documentation techniques, and explain how they affect patient care and physician liability.

Target Audience

This course is designed for physicians who want to improve patient safety and reduce medical errors by confronting and addressing issues relating to documentation and record-keeping. 

Methodology

This is a self-study course consisting of text and CME test.  Physicians should read the course content, answer the test questions and submit the answers for scoring.  Those who complete the course with a score of 80 percent or higher will earn a maximum of 5 AMA PRA Category 1 creditsTM.
